National Disability Insurance Scheme (NDIS)
Our organisation is a registered provider that delivers supports to participants of the National Disability Insurance Scheme (NDIS). As a provider we deliver supports in accordance with the participant’s plan. The NDIS is a way of providing community linking and individualised support for people with permanent and significant disability, their families and carers.
In Home Care can work with individuals to provide a range of supports needed to participate in and contribute to their social and economic life to the extent of their ability and to support them to exercise choice and control in the pursuit of their goals. We can assist with the planning and delivery of supports.
We offer the following services:
- Early Childhood
- Community and Registered Nursing
- Daily Personal Activities
- Early Intervention
- Household Tasks
- Participation in Community, Social and Civic Activities
As a provider under NDIS we understand that everyone's needs, preferences and aspirations are different. If you are a person with a disability and you meet the access requirements you can become a participant in the scheme. As a participant, you will be able to access the NDIS individualised planning processes to identify the reasonable and necessary supports you need to enable you to achieve your goals. To get an idea about whether you meet the access requirements, you can use My Access Checker, available on the NDIS website.
Please note that if you wish to become a participant under the National Disability Insurance Scheme, you must complete an Access Request Form. For more information about the National Disability Insurance Scheme, please visit: www.ndis.gov.au or contact us direct Ph: 03 9455 8989
